I've never had Botox but I'm guessing the needle is much smaller than a needle that collects blood. Have you ever had a shot in your mouth to numb it for dental work? I think those hurt/pinch but are tolerable. To me, that's more of an achey pain. And I feel like if someone can bear that then they can tolerate Botox. But, I am 42 - Sam needed it done years ago. It's sure not in her genetics to get those wrinkles, either. That just shows she didn't/doesn't take the time to care/protect her skin. We all saw past pictures of her brown and orange as shit tan from the fake baking she did.
Sam better read up on it - if she is expecting immediate results, she'll be disappointed. It can take up to 14 days to see a difference.

If you get it done by a reputable Doctor (you know, not at a nail salon!) then I bet they could do topical anesthetic. But I'd imagine there'd be some pain under the surface.
Maybe start small and see if it's bearable for you. I wanna get it done and I'm not scared. I'd prolly be a tad nervous cuz it's needles going in my face but eh. I'm sure there are worse things.
